Your information: We keep a file with your name, address and that you have agreed (or consented) to us having the answers to your questions. This file has a password and only people who work on this study can see it.
The information about your name and address is not kept with the information you have given us when you answered the HAPPEN Survey. They are kept in separate places.
The answers you give us in the survey are kept without your name and address. This means we can not tell they came from you. These answers are kept protected so that only people with a special electronic key can see them.
You can change your mind. We ask you if you are happy to answer questions about your health. If you say yes but then change your mind this is fine. If you do change your mind you can tell us (or your teacher) and we will remove all your information. You can change your mind any time you want.
