1. Contact Parents/Guardians with the information sheet by sending the following link through text or email to those who have children in year 4, 5 or 6. This link should ideally be sent 2 weeks before running the survey:

After you have sent the link to parents, we will inform you of any pupils whose parents do not want their child to take part. Please keep an eye on your emails as these will be sent to you.
2. Show pupils the children’s information sheet below. This should be done at least a day before completing the survey and pupils should be provided time to ask any questions.

4. Deliver the online survey, which takes about 20 – 30 minutes to complete on computers/tablets.
PLEASE NOTE:
The survey asks for children’s postcodes and you may require a list from your school reception if pupils do not know
5. Receive your school report back at the end of the month. If you are unable to complete the survey with all pupils within the month, please let us know to avoid incomplete reports.
